Important Practice Update

 
 
Dear Patients,
 
You may have heard that dentists are reopening from the 8th June 2020.  Wonderful news indeed, but please do not anticipate an immediate return to ‘normal’. There are a number of facets to this reopening that may surprise you. We heard we were to reopen at the same time as you, when Boris Johnson announced it for the 8th June. The dental governing bodies had given us no suggestion of any anticipation of opening in June at all.  The CDO’s (chief dental officer) statement was issued shortly after the announcement and was, frankly, somewhat vague. We only received practical advice on changes to our operational procedures and an altered approach to risk assessments this Monday afternoon (1st June).  We have a lot of work to complete in order to adjust but I am confident that despite the short notice we will be ready to begin our phased reopening by Monday. These broad political statements over simplify complex situations and raise public expectations without any mention of the challenges or nuances and are frustrating to say the least! This is at least my fourth attempt to write an update due to constant change, so I do apologise for the delay. Now that we have been given some ‘rules and recommendations’ on how we practise in this new challenging environment whilst keeping you and us safe. The team and I are zealously studying and determining how to implement the new protocols within our setting. By the end of the week I intend to have trained the team and implemented these changes and protocols, in order to introduce a phased reopening beginning on Monday.
 
A key element of our advice is that whilst at a high COVID-19 alert level, appointments where the risk of generating an aerosol of your saliva is high should be avoided, i.e. spraying your saliva into an invisible cloud in the air caused, for example, by drilling, ultrasonic scaling etc. The alert level refers to the UK government 5-level alert description, we are currently at level 4 which is ‘severe’. This means there are many treatments we still need to avoid. There are exceptions to this, including urgency if you are in pain, in which case we will be using additional cross infection control methods to protect you and us, principally this involves increased PPE, leaving surgeries ‘fallow’ for 60 minutes after such a procedure and using rubber dam isolation and high volume suction as extensively as practicable.
 
Prior to our shock announcement I had already started to prepare the practice and consider how to socially distance staff and patients within our setup. I decided to invest in some PPE, basing decisions around what has been used at the urgent dental care hubs as the shortage was highly apparent and the worsening of that shortage when we came to opening seemed inevitable. During the last week Kim, our practice manager, has become a qualitative face fit tester, which means we can now ensure that our new filtering face masks are fitted as per the requirements and that as our supplies are likely to vary we can continue to check fit safety should the brand change. This has proved useful to get us a little ahead however, PPE is in high demand and in short supply, as you will have heard on the news.  The supply issues have also meant that costs have risen significantly. I consider your safety and that of my staff absolutely paramount and we will follow all advice to the letter. 
 
We have always followed very strict cross infection control policies and will now be attaining even more stringent requirements. The difference is that we now need you to ask you to follow some policies too. We will make these available on this website very shortly, but we will run through things with you prior to your appointments too.  One that is applicable to everyone however, is a polite request that you, please, do not ‘pop in’ to the practice. We are operating a ‘closed door policy’ and will not be using the waiting room in order to minimise contact and support social distancing.  If it is an emergency please call. If it is non-urgent please either call or email. This closed door policy will be the case until the COVID-19 alert level drops to 1-2.  
 
When you attend the practice the system will be very different. We will triage you beforehand over the phone both in terms of your complaint but also for symptoms of coronavirus. If you have an appointment stay in your car when you arrive. We will run you through our full pre-appointment information when you book in and will email you with any paperwork required prior to your arrival, please return this promptly as we will not be able to see you without it. We will be dressed in a considerable amount of PPE, please don’t let that put you off it’s still just us underneath this brave fashion statement!
 
If you are on my list of people with a problem we will contact you, thank you for being patient.  If you are on my list of those who have had an appointment cancelled, again, we will contact you and thank you for being even more patient still.
 
Lastly I would like to thank all those of you to whom I have spoken or emailed who have been so overwhelmingly supportive. The practice wouldn’t survive without you and we will make this new situation work safely for us all and continue to look after you to the utmost of our abilities.
